Planning For The End Of Life

We provide respectful, caring guidance through the end of life process.

In our culture, we have difficulty discussing the final stages of life. We all know it will happen, but it’s still a difficult discussion for many. Even so, it is one of the most meaningful conversations a family can have. I’ve been in many situations where the topic of end-of-life planning has come up. Most have been in a clinical setting, but others have been with family and friends at home. Reactions to the topic vary from fearful looks to crossed arms to comments like, “I know I need to, but I just haven’t done it.”

Putting a plan in place provides peace of mind for you and your loved ones. It’s a gesture that expresses your wishes and values. It guides your family and loved ones during a time that will be overwhelming for them. It also communicates your desires to the healthcare team so that they can honor them. If you cannot speak for yourself, everyone will know your wishes and how to carry them out. You’ll rest easier knowing everything is in place as you’ve planned.

It’s important to consider that you may not be terminal at the point you cannot communicate. For example, you may have been involved in an accident, had a stroke, or have other medical conditions preventing you from participating in your care. It’s equally important, maybe even more so, to have a plan in place now.

Larimar Patient Advocates will provide respectful, caring guidance through this important process. When you are ready, we can:

· Open the conversation about end-of-life decisions and how to formalize your wishes

· Provide education on advance directives and healthcare powers of attorney, along with resources for completing these documents

· Discuss palliative care and hospice options when needed

· Provide resources for family support through the grieving process

We help clarify medical information, treatment, and services for their patients and caregivers. Larimar Patient Advocates begin with a thorough patient assessment to glean relevant information such as medical history, medications, and symptoms as well as patients’ concerns and questions about their conditions.

One of the main benefits of patient advocacy, especially in complex cases, is that it helps prevent readmissions to the hospital.
