Care Coordination
Navigating a health crisis or managing multiple medical issues can be challenging, but Larimar Patient Advocates is here to help. Our approach begins with a thorough assessment, gathering information about your health history, medications, symptoms, and concerns to get a complete understanding of your needs. We then provide a range of services, including reviewing medical reports, medications, and providing educational resources to aid in decision-making.
In addition to these services, we offer practical support to help you manage your health. This includes attending doctor's appointments and providing visit summaries, assisting with finding care facilities, coordinating with discharge planners, communicating with your care team, helping with insurance and billing, arranging transportation, and more. Our goal is to empower and inform you throughout the process and make managing your health less stressful.
Medication Management
We work closely with our clients and their healthcare providers to develop a comprehensive medication plan that is tailored to their unique needs and goals.
This includes reviewing current medications, identifying potential interactions or side effects, and ensuring all medications are being taken as prescribed. Our patient advocates serve as a liaison between our clients and their healthcare team.
Senior Services
Aging in their own home is a priority for many seniors and their families. Being in that familiar space with cherished memories supports the health and well-being of our elders. However, for families who live out of the area, this can be a challenging situation. Larimar Patient Advocates understands this and is here to support seniors in aging in place.
We offer a range of services to help ensure the safety and comfort of seniors in their own homes. These include scheduled check-in visits, medication system checks to ensure medications are being taken correctly, safety inspections and resources for needed adjustments, and assistance in connecting with community resources for personal care, meals, transportation, and social engagement.
With our help, seniors can maintain their independence and enjoy a high quality of life in the comfort of their own homes.
Transfer Assistance
Transitions from one level of care to another can be complex and stressful. At Larimar Patient Advocates, we understand the importance of a smooth transfer and are dedicated to ensuring your safety and well-being throughout this process.
Our team will work closely with the discharging and receiving facility to facilitate the transfer, and ensure that you and your loved ones fully understand the discharge instructions. We also provide practical support to ensure a seamless transition, such as assisting with making follow-up appointments, attending appointments and providing visit summaries, assisting with finding short or long-term care facilities, home health care, and durable medical equipment, reviewing changes in medications, conducting a home safety inspection prior to discharge, and arranging transportation to appointments. With our support, you can feel confident and prepared during this important time.
End Of Life Services
End of life is a sensitive topic, but it is one of the most crucial conversations that families can have. Having a clear plan in place not only provides peace of mind for you and your loved ones, but also ensures that your wishes are respected if you are unable to speak for yourself. At Larimar Patient Advocates, we are here to provide compassionate and respectful guidance through this important process.
Our team can help initiate conversations about end of life decisions, including living wills and healthcare power of attorney, educate you on advanced directives and provide resources for completing one, discuss palliative care and hospice options to assist in decision-making, and provide resources for family support during the grieving process.
We understand that this can be a difficult journey, but with our support, you and your loved ones can feel confident and prepared for this next chapter.
Services We Do Not Offer:
It is important to note that we do not offer legal counsel or participate in legal proceedings. We also do not provide hands-on care, such as in-home or hospital assistance. Our role does not replace that of your medical doctor and we do not diagnose or prescribe medication.
